Weight management intake packet
- Basic intake: demographic details, height, current weight, goal weight, and prior program history
- Medical history and safety: chronic conditions, thyroid cancer or MEN2, pancreatitis, gallbladder disease, pregnancy status, allergies, and GLP-1 exposure
- Lifestyle and goals: meals, snacking, exercise frequency, sleep quality, stress, biggest challenges, and why weight loss matters
